Abbandonato il suo lavoro di patologa forense a Charleston, in South Carolina, Kay Scarpetta viene convocata dal dipartimento di polizia di New York per esaminare Oscar Bane, un paziente dell'ospedale psichiatrico di Bellevue accusato di omicidio. Costui ha espressamente fatto il suo nome e non intende parlare con nessun altro.
